Instructions for Preparation of the Map Accompanying the Mining & Reclamation Application

When submitting an application to the Department the following plan should be included with the Surface Mine application. Click on the following for further directions

Short Form Mining & Reclamation Plan (sites under five acres)
Long Form Mining & Reclamation Plan (no explosivies)
Long Form Mining & Reclamation Plan (with the use of explosives)
