IMPORTANT - Updated Completion of Remedial Action Form

Dear School Representative,


Per State regulations (COMAR 26.16.07.11), all remedial measures conducted by the school to address elevated levels of lead in drinking water should be posted on the school’s website no later than 30 school days after implementation, including all actions taken and associated dates. 


In an effort to assist school personnel in meeting this regulatory requirement, . the Maryland Department of the Environment (MDE) has recently updated the Completion of Remedial Action Form (linked here) to have schools certify that the remedial measures have been posted on the school’s website, including the date on which the necessary information was posted. The update, on the ‘Page 4 Certification’ tab, includes the addition of the ‘Certification IV’ entry (column N), the month/day/year entry (column O), and the month/year entry (column P) if only that information is known.


If your school is currently in the process of filling out the Completion of Remedial Action Form using the previous version, you do not need to re-do the form using the updated version. If this is the case, please notify MDE of this when submitting the Completion of Remedial Action Form to MDE via email (testresults.leadschoolwater@maryland.gov). If you have not yet started to complete the form, please use the updated version.  As a reminder, to ensure that the most up-to-date versions of all forms are being used, MDE strongly encourages school personnel to obtain forms directly from our web page rather than using older versions of the forms.
